LSS/Housing support – Support in everyday life

What can we do to reduce the gap between needing physical support and living an independent life? How can social care staff provide digital support to people living in their own homes?

These were the questions from Sundsvall Municipality to Cuviva. The result was a tool in Cuviva’s platform, created by staff and users who understand the needs. This tool is used in Sundsvall’s Individual Support unit as a supplement to face-to-face contact, helping to increase independence and support more people with the same or fewer resources.

The Solution

Support staff enter a plan with agreed tasks in Cuviva’s secure digital platform, linked to the user. Users can access a mobile app that shows their relevant tasks. The app can also notify users of planned tasks. Together, staff and users can follow up on the tasks through visits or the app’s chat and video functions.

Results

Sundsvall Municipality reports that the tool helps users feel more independent while saving staff time.
