The foundation is built on interprofessional teamwork around each individual case. The digital health and care team coordinates with the collaborative efforts and is responsible for ensuring that both technology and continued support are smoothly integrated. This creates a clear structure – “one way in” – for the citizen and provides reassurance that the right competence is always close at hand.

The digital interventions are carried out, in accordance with the Social Services Act and the Health and Medical Services Act, wherever possible by the interprofessional team. The goal is not only to complement but also, where appropriate, to replace physical interventions with digital alternatives. In this way, resources can be stretched further, and care, support, and services can reach more citizens.

To facilitate the work, the municipality sets up digital teams that can be contacted when support or technical help is needed. A broadly implemented “digiphysical” approach is introduced to ensure modern and sustainable care that both strengthens the work environment and increases security for citizens.

With digital teams within home care, municipal healthcare, and special housing, the foundation is laid for future-proof care and support. The focus is on creating good working conditions for employees and providing citizens with the security and continuity they need.
